Patient pre-consultation checklist
Prior to your consultation, please make sure you bring the following
> A valid referral from your GP
> Imaging such as a CT scan, MRI or X-ray
> Medicare card
> DVA card
> Private health insurance details
> Fill out any required forms/patient questionnaires
Patient pre-surgery checklist
> Make sure your referral is current, otherwise costs associated with your surgery care/aftercare may not be able to be claimed through Medicare.

> Complete all of your pre-operative scans and tests such as CT scans, MRI, hearing tests, blood tests, X-rays etc...
​
> Complete any required hospital admission forms or medical clinic patient questionnaires.
​
> Make sure you have received and signed any quotes or cost estimates for your surgery and you have organised any required payments.
​
> Don't forget to declare any supplements, vitamins or medications.
​
> During your consultation notify Dr. Cavada if you intend to travel before or after your surgery.
​
> You will be notified prior to surgery admission times and fasting requirements.
